Shot hours: Tuesday & Thursday from 9:00am-4:30pm. We are closed from 12-1 for lunch. Shots can also be scheduled by appointment only for Monday afternoon 1-4pm or on Friday mornings 8-9:30am on a trial basis. Please arrive 5 minutes prior to your shot time to complete your forms.

For patients who are scheduled to have their allergen immunotherapy shots during the holiday or closures, please contact the office to re-schedule your allergy shot.

If you're having a medical emergency, call 911. If you are having an urgent medical matter, please contact your primary care physician's office or go to the nearest Emergency room or Urgent Care Clinic for medical attention.
FLU VACCINE TESTING FOR EGG ALLERGY PATIENTS: we are now taking reservations for our patients who are allergic to eggs and need testing for the influenza vaccine. You must be an established patient. Please call our office and reserve a vaccine for administration. Be sure to indicate your child's age as well as whether or not this is their first time ever receiving the flu vaccine as 2 doses would need to be reserved. $30 will be charged to your credit card if you cancel or no show as we are holding the vaccine for your child. Appts are dedicated to Friday mornings only beginning October 14-December 16, 2011.
SICK POLICY: Please Note: if you or your child are ill, for example has an upper respiratory illness (cold, RSV, croup, cough, or fever), please reschedule your appt. We have patients who are immune suppressed due to immune deficiencies and are more prone to serious complications even from the common cold.
SCHOOL FORMS: please give us 2 weeks in order to complete them. Include all of your child's infomation as well as a self addressed stamped envelope, fax number to the school and we can directly fax them for you, otherwise we will call you when it is ready for pick up. The first set of school forms per year are complimentary however if this is your 2nd set of forms within the past 12 months then there is a $10 fee. Please include a check for $10 with your child's school forms.
