UPDATES: Recent voluntary recall of Zyrtec & Benadryl are for the liquid forms only.  You can substitute with chewable forms of the medication or try Cetirizine Syrup (generic for Zyrtec) or Claritin Syrup.

School or camp forms:  please allow 2 weeks to complete these forms. The first set of forms for the year is complimentary, subsequent forms will be charged $10 per set. Drop off or mail your forms and be sure to include a self addressed stamped envelope if you would like them mailed back otherwise we will call you when they are ready to be picked up. Please complete on the form, your child's name, date of birth, medication for which you are requesting that we complete the form.  Make sure that there are adequate number of forms (one per medication).

Shot hour change:  Tuesday & Thursday from 9:00am-4:30pm.  We are closed from 12-1 for lunch, except by appt.    All shot patients, please arrive 5 minutes prior to your shot time to fill out your questionnaire and perform a peak flow, if needed. if you miss your shot appt, please call and schedule another on Tuesday, Thursday.  Shots can also be scheduled by appointment only for Monday afternoon or on Friday mornings when patients are being seen.

 No shots on Monday August 16th. Sept 2nd and 3rd modified hours, check with staff.

CLOSING:   We will be closed Monday, September 6th in observance of the  Labor Day Holiday. 

EFFECTIVE AUGUST 1, 2010, WE WILL NO LONGER PARTICIPATE WITH UNITED HEALTHCARE.  FOR OUR CURRENT PATIENTS WITH UHC, THEY HAVE THE OPTION TO TRANSFER THEIR CARE TO GEORGETOWN OUTPATIENT CLINIC WHERE DR. BAILEY SEES PATIENTS ON WEDNESDAYS, GO OUT OF NETWORK OR TRANSFER TO ANOTHER PHYSICIAN IN NETWORK.  WE WILL FAX NECESSARY MEDICAL RECORDS TO THE NEW ALLERGIST AT NO COST. (HOWEVER MAILING MEDICAL RECORDS WILL COST $30 PLUS FOR ADDITIONAL PAGES OVER 10).  PLEASE MAIL A WRITTEN REQUEST WITH THE PHYSCIAN'S NAME AND FAX NUMBER AS WELL AS A WRITTEN REQUEST OF MEDICAL RECORD TRANSFER. ALLOW 2-3 WEEKS FOR THE RECORDS TO BE REVIEWED AND FAXED. THANK YOU.

SICK POLICY: Please Note:  if you or your child are ill, for example has an upper respiratory illness (cold, RSV, croup, cough, or fever), please reschedule your appt.  We have patients who are immune suppressed due to immune deficiencies and are more prone to serious complications.
